Special Education Records Notice
In accordance with ARS §41-1351, the retention period is four (4) years for Special Education Records. Sage Academy will destroy Special Education Records of students who have been dismissed or withdrawn from Sage Academy Charter School for four (4) years or more from current academic year. Prior to the destruction of records, you have the right to review the records and obtain copies of any information. To obtain records you must be the parent or legal guardian of the student or the student (age 18 or older). Please indicate your request by contacting the Director of Special Programs at 602-485-3402 before June 30. If you do not contact the Director of Special Programs on or before June 30, all records for four (4) years older than the current academic year will be destroyed. information for all employees who provide instruction to pupils pursuant to A.R.S. 15-183 is available upon request.
Public Meeting Notices
Pursuant to A.R.S. §38-431.02, Sage Academy, Inc. hereby states that all physical notices of public meetings of Sage Academy will be posted in the school office located at 10220 N. 25th Ave Phoenix AZ 85021. This location is open to the public Monday through Friday from 7:30am to 4:00pm. Electronic Notice of Public Meeting will be posted on our website. Such notices will indicate the date, time, and place of the meeting and will include an agenda or information concerning the manner in which the public may obtain an agenda for the meeting.
